Sarasota County and Longboat Key have been selected as the exclusive location for a pilot program implementing the Johns Hopkins Guided Care model in home care, a new approach to managing complex health needs for high-risk older adults.
Sarasota-based Youthful Aging Home Care will welcome Chad Boult, MD, MPH, co-developer of the Guided Care program and lead author of landmark research published in the American Journal of Managed Care, to present two educational sessions in October to help introduce the pilot program to the community.
The presentations will take place Oct. 28 at the Sarasota Yacht Club at 10am and Oct. 29 at 10am at Temple Beth Israel on Longboat Key.
Youthful Aging Home Care has been chosen as the exclusive provider to deliver this pilot program, making Sarasota County the first community in the region to offer residents access to this evidence-based care management system.

Boult’s research showed the Guided Care model reduces hospital readmissions by 13 percent, skilled nursing facility days by 26 percent, and home health episodes by 29 percent compared to traditional care approaches. The model employs specially trained nurses certified by the Institute for Johns Hopkins Nursing who serve as “nurse guides” creating what researchers call “an invisible safety net” connecting patients, families and physicians.
The Guided Care approach focuses on proactive care management for patients with high-risk chronic conditions and complex health needs, stopping health crises before they start and preventing others from escalating.
Boult previously served as senior adviser to the Center for Medicare and Medicaid Innovation. His work has focused on developing innovative approaches to primary care that improve quality while reducing costs for older adults with multiple chronic conditions.
The October presentations will detail how the pilot program will operate in Sarasota County and Longboat Key, providing families with evidence-based strategies for comprehensive care management that enables aging in place.
“Traditional home health care helps temporarily, but doesn’t connect the dots the way comprehensive care management does,” Lieberman noted. “We are excited to demonstrate how proactive care coordination can transform outcomes for our most vulnerable residents.”
The presentations are open to anyone interested in helping themselves or loved ones age in place, as well as medical and emergency professionals. Reservations are required for both events.
